I found a different form, set up sort of the same way, but lacking in features. It is Member Application vs 2.14 and I had no problems installing it. (make sure that you get the one that is made for RN) It is encoded like the other one with ioncube, so I don't know if anyone can change it. I would like to find out who made it (if they are still around) and see if they can add a date block to the dropdown menu part of it. If anyone knows who made this, I would like to be able to contact them, if you can send me a message. Or if anyone knows how I can add a date box (m/d/y) with a little calendar icon next to it under the questions area that would be great. I seen that dreaded corpse had one on his, but he was running evo.

GRRR.....LOL. I thought it was a different person because it was named something else. If someone could come up with something like that, it would be great. However, on a good note, that version I mentioned I had no problems installing and using, and it works 4.0! Now if there was just a way to add that calendar to it, I would be in there like swimwear! Smile
