I'm looking to hire someone to make me a simple module for my nuke site.

Basically all I need is a module that is a form that propagates a spreadsheet that is viewable by my members.

There are six fields and I need the members to be able to update their information as needed.

I think this should be very simple and very straight forward. Please PM me if more info is needed and also to provide a price quote. I would like to get this done ASAP.
