How do I create a form submittal page?

Is there some sort of standalone block or some block or module I can download that will give me like a user entry form with a submitt button going to the email address I setup?.

I'm looking for something like this: [ Only registered users can see links on this board! Get registered or login! ]

This is not a nuke site I'm just displaying the idea.

Anyone got some info on how I can get something like that working in my nuke 7.9 site?

There are any number of free form generator software out there for you to make a custom form BUT you need to be very careful when using an open form like that to ensure it cannot be used to send spam or indeed that you end up getting tons of spam yourself.
Some of the gaming type sites have found out the hard way - you don't have to.
If you are familiar with PHP you could modify the feedback module but the simplest thing is to place a prominent link on your home page to a zipped Word document and have them fill it in and email it to you - (put the email address in the document).

I would also suggest that if you are handy with PHP you could copy the feedback module.
