Ok, I figgured this one out. This was because the filename contained "(SD)" as a part of the title. I was able to rate other files, so I deducted the ony real difference was the Title of the Download.

Here's the real question now. How can I make sure that no users put any parenthisis in the title. How would we "ban" their use from the Download module ? Or is there something else ?

You can change the rules of your download submissions and add Please do not use parenthesis in your titles. Or you can change them yourself manually..

You could use str_replace for the title but when its part of the filename its gonna be a pain in the arse anyway. So I think blith's suggestion might be the easiest and most effective in the long run.
