Youth in need database module

1. Accept a picture (minor requirement)
2. Name
3. Address (needs a feature to hide / lock)
4. E-mail (needs a feature to hide / lock e-mail)
5. A data field to accept text in free form
6. A field that will allow the user to make multiple selections: (clothing, educational support, toys, books, school supplies, etc.
7. flexible enough so that I can make a translation into French.
8. searchable by keyword
9. Only registered users

Basically it will be used to store info for youth in need. People who want to help will access the module, possibly contact us for more info before they can see all data pertaining to that teenager / adult.

If you know anything that can be used for such purposes (closely enough with proper e-mails security) please let me know.

I don't know of any existing script but if you want to discuss contracting it out to me, let me know. You do not have to respond to this post - it's just meant as an FYI.
