Mysql Tables Fail To Install

Well it appears I jumped the gun. I just tested my installation of Sentinel and it will detect an attack and email me, but it will not block the IP of the attacker. I read the threads on how to go about unbanning oneself and then looked for the table in phpmyadmin . It appears when I ran nsnst.php no tables were created, so I went ahead and tried to run it again. When I run it every table creation fails. Here is my info :
