What do you want it to do, exactly? Depending on how you answer that, you might be able to just make do with a Content page. |

Yes, and I believe admins don't get filtered either, so you could probably put a form in content. You could also direct them to the Feedback module with a link. |

No, it is licensed under the GPL. It is perfectly fine to modify and build from his work. Just preserve the original copyright in your new code and give him credit.  |

i changed the index.php, uploaded it to my challange_us module, and saved the info but when i try it on my site it says:
"Sorry, that module file was not found." |

You need to make a new directory for your new module, then activate it in the admin control panel / modules. See also the PHP-Nuke How-To which has some info on making your own modules. |
