What do you want it to do, exactly? Depending on how you answer that, you might be able to just make do with a Content page.

Yes, and I believe admins don't get filtered either, so you could probably put a form in content. You could also direct them to the Feedback module with a link.

the feedback module is copy right by raven nuke.. isnt that illegal to modify it?

No, it is licensed under the GPL. It is perfectly fine to modify and build from his work. Just preserve the original copyright in your new code and give him credit. Smile

i changed the index.php, uploaded it to my challange_us module, and saved the info but when i try it on my site it says:

"Sorry, that module file was not found."

You need to make a new directory for your new module, then activate it in the admin control panel / modules. See also the PHP-Nuke How-To which has some info on making your own modules.
