I have since removed SPChat and changed database and CPanel passwords.

Why not password protect the admin.php file and the /admin folder via the OS and utilize SSL when logging in as Admin?

What may have happened is that the hackers ftped or telneted an altered index.php and overwrote your original index file. Do you allow anonymous ftp to your webserver?
