I am looking for a way to require prospective member to fill out a simple questionare prior to being approved, and if approved this information be added to the db. I am currently using Approve Mmbership module so I would like to ensure the answers are visible to the admin approving the request. I hope I have been clear.

I would prefer an answer as to how, maybe a few why's so I can not only get it done but learn from it.
