make a template in content?

okay since my review module wont work well, ive decided to use content module for my reviews, is there a way to make a template for it, like

name of program:
website:
and score:

You could make changes to the content module to handle this, but you might want to consider one of several other reviews modules that are available.

can you think of other review alternatives?

i found NDReviews but there are bugs and it doesnt work well
and I tried looking for MReview
but i cant seem to find it , the site is down
are there others?
