A few problems, help please

I am running PHPNuke 7.5 and Sentinel (newest version). I find I'm having a few troubles.

First:

I cannot log out of my account. I can't log out of my admin account or my user account. Whenever I click the logout button, I get one of two errors popping up at the top of the screen:

And it won't log me out.

This is also causing me to be unable to add/delete/modify blocks, etc. And could someone please tell me what the CVS is in this version of nuke? I see the folder in my domain and I see an option for it on my site. What is it?
