@ kguske - Yes Akismet is an external service (free API key required) but so far I have no issues with except the very rare occassion when due to service load it won't vet the data and falls back to a "Your comment could not be posted, please try again later" but even that wouldn't be a problem if you used it on already submitted data like the Comments module or put the data in a 'pending' area. I have only had this happen twice so it is extremely rare.
Regarding blocking referers - a lot of sites like to put links on their sites to higher PR'd sites (SEO 101) in the hopes it will give them an edge. Problems arise though if you have 50 porn sites linking to you, 50 'finance' sites etc etc. It might be good for them (in the short term) but it kills your own PR.
Earlier versions of phpBB were exploited to do the reverse - someone registers, usually with a keyword oriented username and puts a link back to the site they are promoting via the forum sig. They don't even have to post in the forum as search engines will crawl the userinfo
